Welcome to StudentScholars After-School Program! To ensure a safe, supportive, and enriching environment for all students, we ask that parents review and adhere to the following policies and terms:

1. Program Hours

1. The After-School Program runs from [3:30 PM – 6:30 PM], Monday to Friday.

2. We do not charge for late pickup. Children should be picked up on or before  6:30 pm

2. Attendance & Absences

1. Regular attendance is encouraged for consistency and progress.

2. Please notify us via email or phone if your child will be absent.

3. We offer 1 credit a month if you inform us 24 hours in prior to the start of the lesson.

3. Drop-off & Pick-up Procedures

1. Parents/guardians must sign students in and out.

2. Only authorized individuals listed on your child’s registration form may pick them up.

3. ID verification may be required.

 

4. Behavior & Conduct

1. Respectful behavior is expected from all students.

2. Bullying, disruptive behavior, or disrespect toward staff or peers will not be tolerated.

3. Repeated behavioral issues may result in temporary or permanent dismissal from the program.

 

5. Snacks & Allergies

1. Students may bring a nut-free snack. Please inform us of any food allergies.

2. We are a nut-free environment for the safety of all children.

 

6. Medical & Emergency Information

1. Parents must provide updated emergency contact and medical information.

2. In the event of illness or injury, parents will be contacted immediately.

3. In serious cases, emergency services will be called.

 

7. Payment & Fees

1. Program fees must be paid by the 1st of each month.

2. Late payments may result in a hold on attendance or late fees.

3. No refunds are given for early withdrawal unless due to medical reasons with documentation.

 

8. Program Cancellations

1. In case of severe weather, public health concerns, or emergencies, the program may be canceled. Notifications will be sent via email or text.

2. Any cancelled classes by the provider will be rescheduled when possible.

 

9. Personal Belongings

1. Please label all personal items. We are not responsible for lost or stolen belongings.

2. Students should not bring toys, electronic devices, or valuables unless authorized for a specific activity.

 

10. Communication

1. All program updates and announcements will be shared via [email/newsletter/parent portal].

2. We encourage open communication—please contact us with any concerns or feedback.

 

11. Assumption of Risk and Indemnifying Release

StudentScholars staff and instructors will make every reasonable effort to minimize exposure to known risks. However some risk of injury still exists.

I/We understand that in registering my child in the StudentScholars afterschool programs, my child/children will be involved in physical activities and that with any physical activity, there is a risk of injury.

​

I/We do hereby release StudentScholars and its Directors, Employees, Volunteers and Agents, from all liability for damages sustained in consequence of loss, injury, or damage to myself or my child; from all other actions, causes of action, claims, demands or damages of any kind with respect to death, injury, loss, or damages to any person or property including myself or my child arising out of, or connected with preparation for or participation in the programs or activities.

 

12. Transportation Policy​

• Transportation services are available through independent drivers.

• Pick-up and drop-off pricing is arranged directly with the provider.

StudentScholars does not provide transportation services directly. However, for the convenience of families, some parents may choose to arrange transportation through independent drivers or third-party providers.

Please note the following important points:

• StudentScholars is not affiliated with, responsible for, or liable for any transportation arrangements made between parents and independent drivers.

• Any agreement or payment for transportation is made at the parent’s own discretion and risk.

• StudentScholars staff will not supervise pick-up or drop-off beyond our designated sign-in/out procedures on our premises.

• In the event of a delay, miscommunication, or incident involving an independent driver, StudentScholars cannot intervene or be held responsible.​

By choosing to use third-party transportation, parents acknowledge and accept full responsibility for the safety and arrangements of their child’s travel to and from the program.

Authorize on my behalf

I/We have provided a complete and accurate health medication information and permit my child to participate in the full range of activities, except as noted by me in the health medication information section of the StudentScholars registration form. In the event of an accident injury or illness involving my child and immediate contact by the StudentScholars with a parent/guardian can-not be made, I/We authorize and grant permission to StudentScholars staff to secure proper medical treatment and authorize on my be-half all procedures, as deemed necessary by the attending medical professional(s). I/We agree not to hold the StudentScholars  responsible for any costs or injury arising out of an emergency situation.

The StudentScholars may wish to use photographs, images or recordings containing my child’s picture or image for promotional, advertising, public relations and/or informational purposes. Such photographs, images, or recordings may be used or published in  StudentScholars brochures, newsletters, posters and/or website/internet (including social media) materials. I/We hereby consent to the publication of these photographs, images, or recordings, and promotional advertising, public relations or information materials, and acknowledge and confirm that these photographs, images, recordings and materials shall remain the exclusive property of StudentScholars, who shall own all copyright and other intellectual property rights therein.

I/We declare having read and understood the above INFORMED CONSENT AGREEMENT in its entirety and hereby consent to participate acknowledging all the foregoing. I/We also certify that the information provided in this form is, to my/our knowledge, true and complete.
